Assessment and Care Plan
There are many times you just don’t know where to turn or what to do about your elderly family member who are experiencing changes as they age. Housing, physical impairments, home health aides, memory loss, financial matters and other issues swirl around conversations during your work day as well as your off hours with friends and family. The Assessment & Care Plan, which is done by one of our experienced, professional staff, provides an objective overview along with a care plan that supports the assessment and provides detailed long-term care options for your loved one.

What Is It?
The Assessment and Care Plan is a comprehensive document which contains an objective, professional view of the client and their environs. The assessment includes information about the client’s medical, psychological, emotional and social needs. In addition family dynamics and home safety are also addressed as well as the losses endured by our client. The care plan is a road map for the family to follow to ensure safety and a higher quality of life for their loved one. It often provides options for the families as they try to provide a long-term planning for the near and not so near future.

How does this Work?
The process involves one of our staff members, usually a licensed social worker, meeting with the client and speaking with their families or other members of their support system. A visit to their home is part of the assessment, even if they are currently in a facility. Our staff person may need to speak with medical personnel or other health workers associated with the client. The assessment and care plan will be verbally communicated with the contact person and then a written Assessment will be provided shortly thereafter.

PRI Screens (Patient Review Instruments)
The PRI is a document that is necessary for admission into a Nursing Home in New York State. Acceptance into a nursing home is dependant on the PRI score. Our Registered Nurses can complete a PRI for your loved one in New York and Florida. And if you need help with locating an appropriate Nursing Home, or help with the admission process, we can assist with that as well.

Medication Management
Medication abuse is one of the biggest issues among the elderly. Overdosing, under-dosing and taking medications at the wrong times are common among seniors. With the advancement of pharmaceuticals, many seniors take a many different medications. Often, they are over prescribed which can create additional complications. Most home health aides are not allowed (by law) to give medications to their clients. Therefore pouring medications correctly into a pill box becomes the responsibility of a family member or a nurse. In addition, refilling prescriptions and keeping up with changes in medications are also important.

Arranging for and Monitoring Home Health Aides
Finding an appropriate home health aide is one of the most important jobs of a GCM. GCMs also monitor the home health aides on a regular basis. Not all home health aides work out and some need to be replaced for one reason or another. We are aware of what kind of aide might be best for the client and the best agencies providing these services.

Intensive Case Management
Whether you are busy with your own life, live far from your loved one or there is no family at all, we can take care of your loved one as well as their home on an ongoing basis. From doctor appointments and good nutrition to arranging transportation and managing home health aides, we can provide for a wide range of needs. We communicate with the family or responsible parties regarding all matters that may crop up in day to day management.

Assisted Living or Nursing Home Placement
For one reason or another living at home may not be appropriate and another housing option may be necessary. Our knowledge of Assisted Living Facilities and Nursing Homes can help you choose the appropriate housing for you or your loved one.

Entitlements
Many seniors require help with entitlements such as Medicare, Social Security Disability and Medicaid. We can help apply for these entitlements as well as help with the paperwork for insurance claims. Sometimes people with dementia have neglected their mail for a long time and we often help to sort out their personal business.
